While these sample applications will be helpful in a general way, the FY2018 Artist Plan form has changed.

The Major Goal section is now called Project Outcome(s). There are new points to address under Project Outcomes, Actions and Impact on the state and Creative Community. Be sure to refer to the Overview and Instruction document, page 10, when completing an FY 2018 application.
